Conscientia Health Launches Virtual Primary Care Platform Powered by AI
Conscientia Health has launched a virtual primary care service that integrates mental and physical health care through advanced AI technology, announced in a press release. The New York based company provides coordinated care to more than 15,000 existing mental health patients across 73 locations in all 50 states and 20 local sites in New Jersey.
The new telehealth platform allows clinicians to deliver preventive and routine care, chronic condition management, same day illness visits, and behavioral health integration. It uses AI tools for predictive analytics, unified patient records, and real time collaboration among providers.
According to the announcement, the system analyzes integrated datasets to flag potential health risks and streamline workflows, enabling faster and more precise care. With this launch, Conscientia Health expands its hybrid model aimed at reducing fragmentation between mental and physical health services while improving access and coordination nationwide.
