Dimer Health Launches AiME for Post Discharge Care

Dimer Health launched AiME, an AI health agent that helps patients follow discharge instructions, manage medications, and contact clinicians during recovery at home.

Dimer Health announced in a press release the launch of AiME, an AI health agent for patients recovering at home after leaving a hospital. The company says the service has supported thousands of patient chats since its rollout in late 2025.

AiME uses each patient's medical record and discharge instructions to answer questions, explain recovery plans, support medication management, and provide nutrition and exercise guidance. Patients can also share images and schedule virtual appointments with Dimer clinicians.

The agent can flag medication discrepancies, including duplicate prescriptions and conflicting doses, for clinician review. It escalates concerns that require human judgment to Dimer's licensed clinicians and does not replace clinical care. AiME is available through the Dimer Health app and online.
